Business Loan Application

APPLICATION INSTRUCTIONS:

• Complete the Business Loan application
• Complete and sign the Personal Financial Statement on page 2 for each owner of the business. If
   additional copies are needed, make copies.
• Provide signed copies of the last three year's business financial statements and Federal tax returns.
• Provide a current financial statement dated within sixty (60) days of the application.
• Provide a copy of your most recent business account bank statement.
• Provide signed copies of last yearŐs tax return for each owner of the business.
• For an equipment or vehicle loan, provide a signed copy of the purchase agreement or current vehicle
   registration.
• For a line-of-credit application, attach the most recent accounts receivable and accounts payable aging.
• Return the required information to your nearest ELGA office and we will promptly process your application.

PLEASE DO NOT submit any original copies. Any information submitted to ELGA becomes the property of ELGA and will not be returned.

To help the government fight the funding of terrorism and money laundering activities, Federal law requires all financial institutions to obtain, verify and record information that identifies each person who opens an account. What this means to you: When you open an account, we will ask for your name, address, date of birth and other information that will allow us to identify you. We may also ask to see your driver's license or other identifying documents.
